Insert Image Popup fails to fully load

The Problem: When anyone uses the quick-reply to insert an image, the popup that usually appears never fully loads such that someone can upload/attach an image.

BTW - I just upgraded and I get the same error: "SFM_Feb2010" is based on the Defalut Vbulletin 4.0.1 template.

I should ad: On my MacPro I'm running Leopard and FireFox version: 2.0.0.12 and Safari 3.0.4 and both of these applications treat the page the same way.. the buttons don't work.

**Disclaimer - it is entirely possible it's my system which is causing this problem, ie.. the browser.. however I have checked this on a PC and my Mac and I can't seem to get it fixed without some help....
