If you're using InnoDB tables, use drop+create instead of truncate/delete - the latter is extremely slow on large tables such as yours.

On MyISAM, truncate should be more or less instant...

The only real reason for attachments in the DB is ease of installation+compatibility, so it makes sense to have it as the default or remove it entirely.
