Custom skins won't be touched. If you're worried, just backup the skin you've been working on. If anything should go wrong, you can then easily upload your style after upgrade has completed.

First question: upload the old database, upload the old files; done. (no upgrade required)

2nd: You can go straight from 4.0.4 PL1 to 4.0.6 (Although not released yet)

They may allow you to download the database if you ask really nicely, or they may request a small fee. Can't hurt to try.

Yes. As long as only one domain has the software installed (One instance of the software installed). You can redirect as many domains as you like....
