Authorize.net approved Paid subscriptions are showing as failed

Sorry for the dumb question. Where do I specify the variables
i.e {1} is $username
{2} is $subscription_title etc.

Payments via Authorize.net are going through and user is getting charged. However the changes are not reflected in the vBCloud database. It shows as failed in the transaction log. The details in the failure...

I moved from another host to vBCloud and getting the following errors.

• New members that are registered are getting banned right away
• Users that change passwords are also banned

I moved from another host to vBCloud. Emails sent to users do not have the $vboptions filled out anymore. See example email below.
Users are not able to extend their registration due to this....
