you will get a php error if you were putting the set-cookie in the wrong place, so it probably isnt that... you need to check how PHP is set up on the server your script is running as it might be setting...

also if you do use require make sure your library file has the line
1;
at the end. Other wise you get server 500 errors if i remember rightly and it is not the easiest...

Im not sure if ive got exactly what you want to do right....
I dont think that you can inlclude an html file in php... it still has to be named .php even if it does not have php in it......
