IE can be awkward. The best way to get the icon to always show in the address bar is to add the following to the head of your (X)HTML:

Check you are showing hidden files. Open My Computer and go to Tools > Folder options > View and make sure 'Show hidden files and folders' is on
After that, you could upgrade...
