We get several obviously bogus requests for membership each day. A frequent extension is .ru. Are there people that do this, or is a bot somehow defeating the image reading step in registration?

Simple things to do are use the Captcha as stated, ban those email domains that are spammers, use the Date of Birth requirement already present in VB registration, this seems to stop the bots.
I have also added some of the mods from VB.org that deal with bot registrations, and since doing so have seen over 60 attempts to register by bots fail.
This is the Track Guests list which helps see who is visiting

This is "No Spam" and is an extra Captcha device that sets questions to be answered.

This is called "ISBOT" it checks the time taken to fill out the registration and if it is done faster than a human can type then the registration fails.

So bots trying to register have to complete the form, complete 2 Captcha devices, give a DOB and are checked for speed. The Track Guest list shows me what they have tried to do and lists the username they try to register with.
