I got an email from a user who claims to have tried to log in/registered and received a banned message. However, their IP isn't banned. What could caused this?

Check your Usergroup permissions and make sure that "This Usergroup is not a 'Banned' Group" is set to 'Yes' for the groups that you do NOT want banned.Steve Machol, former vBulletin Customer Support Manager (and NOT retired!)

Check to see if their e-mail domain is also listed as part of your banned domain list. For instance, if you have the following in your ban list:
iambanned.com
youarebanned.com
wearebanned.com
and the domain he is trying to register with is:
yesyouarebanned.com
the youarebanned.com may be causing the problem. If that is the case, preface it with "@" in the ban list.
