When users or I try to edit a thread title, the edit doesn't show up on the forum index, just the first post. How do I edit a title and have it show up on the forum index?

Sounds like you are trying to do it via the edit button. This method will only edit the title in the post.
To change the actual thread title you need to open the thread and then select "Edit Thread" from the Thread Tools dropdown menu in the navbar.
Alternatively you can edit a title by double clicking the thread (when viewed as a list in forumdisplay.php). This will highlight the title and you can edit it from there. Then single click to save the changes. -

ok, one more question. That made it so I could change the thread title, but what about users editing their own titles? That option doesn't appear to be available in the drop down menu.Comment

I think that is based on "Time Limit on Editing of Thread Title" setting. So if your member is trying to edit the thread title in post #1 and the limit has expired, then the change will not be reflected when viewing the forum titles, but the post itself will show the change.
I wish the ability to create post titles was completely removed, posts should not have titles, only threads should have titles.Comment

As Andy said its based on a time limit for editing.
Admin CP > vBulletin Options > Message Posting and Editing Options:
Time Limit on Editing of Thread Title
Specify the time-limit (in minutes) within which the thread title may be edited by the user who started the thread.Comment

I've already done that, I've set it on zero. ((shrug))
should I set it for a really high number instead? I thought zero disabled the time limit.Comment
