Make a template....

The template I want to make is an affiliates page where I exchange my sites domain name with other sites and advertise their sites here.

So, how do I make a new template with the same "blackwillow" skin and have a link at the bottom of the page along side options like: Contact Us - Admin - Mod - Archive - Top?

I am new to website designing and coding, so if you guys could keep any help simple, that would be great.
