Saturday morning I woke up to my forum not being able to be accessed and found several thousand lines like the following (from this morning) in my MySQL error file.

Why would this happen twice in such a short time period? Has anyone else had this issue?
System info:
Linux 2.6 kernel
vBulletin 3.0.3 used (yes, I know, old version. few of our hacks aren't in 3.5 yet)
MySQL 4.1.15
PHP 4.4.0
This is causing a very serious issue and needs to be solved quickly.
Thank you.
