I added an additional profile field that I want to have required but I keep seeing newly registered users who don't have that field filled out. I have checked my settings in the CP and it is set to be required. Is there something else I need to change somewhere to ensure that is actually required?

Nope, that should be enough. If you have not installed any hacks, fill out a support ticket at:
http://www.vbulletin.com/members/support_form.php
Be sure to include the login info to your Admin CP and FTP.
