Lost avatar page

When I log on to the admin control page and click avatar modify, add i get a blank white page. How do i fix this, dunno what happened. Please help!!!!!!!!!!

Reupload the original avatar.php file from the zip file. Make sure you upload in ASCII and overwrite the one on the server.
