MySql... PHP ... How the ...

I seem to have a similar problem to many others .. I can't get past the first stage of installation because I have not got permission to make the DB

Now.. How do I make a DB manually.. My ISP is set up with Mysql4 so all should work ok.. The test php works fine and tells me that all is ok.. it's seems to be the adding/making of the db that is the problem..

Any help appreciated , but be specific 'cos I is a thicko!!
