Upload Attachment Problems

I am having some problems in uploading attachements on my forums. After you upload an attachment it says:

No forum specified.

Can someone help me with this?

Have you made any template modifcations?

Yes, I have. I did add the quick reply to my forums, could that be it?

Revert all your files and templates. and see if you still have a problem
