Switch back to the default style and look if it happens if it does not something should have altered the style. Also if you have CSS as file put it temporary back in the database.

I know you said no recent changes but are you sure?

.htaccess in relationship to VBSEO? Double-check your URL setting in AdminCP as well just two off the top of my head. If you are 100% sure no changes were made ask your host if they have done anything recently.

Well the default style newly installed should return the board to normal else this gets rather complicated and the whole style system needs a good kick.

              we've tried the following

              1) created a new default style that isn't the parent of another style
              2) disabled nginx
              3) disabled & uninstalled mod_pagespeed
              4) verified mod_pagespeed was disabled via phpinfo
              5) reinstalled apache
              6) doublechecked site url
              7) disabled all vb hooks

none of the above have helped :\

Your CSS is being delivered inline, like its vBulletin 3, not vbulletin 4.

Infact, all of that CSS is vBulletin 3 css, and NOT vbulletin 4 css.

Reupload the default vBulletin 4.0.5 files and re-run install/upgrade.php

I would advise upgrading to 407/408.
