[Suite] Upgrade error from 3.8 to 4.2

I went through testing this on my test forum twice and didn't have this error. now running the upgrade on the live forum I am getting this...

Column 'contenttypeid' specified twice

Query:

All my mods are disabled, I'm not sure what else to check.

I went ahead and scraped the entire db, not getting that error on the fresh db. all seems well.
