            You said you had 3.8.6- did you mean 3.8.6 PL1, because 3.8.6 itself has a huge security flaw... you would have had to upgrade to patch level 1.

                    If you are still being hacked after doing all of this, then they are most likely doing this by accessing your server. You need to contact your host about this.

                          Also to add, purely on a cautionary basis, modifications should always be sourced directly from vb.org? [rhetorical question there]
                          If by chance they've been repacked, rehashed and supplied via other sources, sometimes in bulk (rar/zip files) from places not unlike *cough* torrent sites *cough* ... then there could be a good chance that some spurious code may have been added. By which time, once installed, the damage is done.
                          Plus the board owner(s), universally speaking, may forget just where certain mods came from.
