People Unable to Register

I've had no new sign ups in weeks - the few users who failed to register have said they are unable to sign up as the screen goes blank etc. Can anybody help?

Thanks - seems I'm unable to login via FTP to check. Very strange.

Hmm.. I also see this file listed in diagnostics - any ideas what it is?
