You can improve the actual online list by editing the module and setting 'show users from this page only' to No.
Most users online doesn't work currently though.

There are a number of issues with "Who's Online." These should be fixed with the 5.1.4 release which should be soon. Already in Beta on this site.Leave a comment:

Who's Online not showing everyone who is onlinely
Like title says I know there are more people on then show up there. Sometimes it says just me and someone is posting simultaneously and it won't show them. This is in 5.1.3 cloud. I also noticed that sometimes it will also say the most people online ever was x amount and later that number will be lower?

I wondered if anyone else if having problems with the "Who's Online" feature not displaying the correct numbers in 5.0.5? I searched the FAQ and other posts but didn't see it anywhere.

None of the regular users can see who is online any more. Myself as admin or mod can only see a small number of users, there are definately more that should be displaying. I'm running 5.1.1a9.Mon 28 Apr '14, 1:16am
